Super nice staff, they answer the phone, got in right away. They fax everything for you to DPS for self-reporting! Also give you a small pocket sized LAMINATED card along with the long form.
Doctor was nice and fun to chat with. My go to place from now on.
It can look kinda deserted but it's not -
